A pill chart is the simplest medication record there is: medicines down the side, days across the top, a tick when a dose is taken. It works on a fridge door, in a folder or on a phone.

How to fill it in

• One row per medicine per time of day. "Metformin, morning" and "Metformin, evening" are two rows, so a tick means one dose.
• Copy the name and strength from the label. The chart is for recording, not for working anything out.
• Tick when taken, cross when skipped, and write why in the margin if there was a reason.
• Start a fresh chart every week and keep the old ones for a month. They are the answer to "how has it been going" at the next appointment.

When a chart is not enough

• Several people in the house each with their own medicines.
• Medicines taken every few hours, or every few days, that do not fit a weekly grid.
• As-needed medicines, where the question is "when was the last one", not "was it taken today".

A list and a log on the phone handles those; a chart on the fridge handles the rest.
