Kids' medicine log for a babysitter or grandparent
Free download, no sign-up: Printable PDF, one page · Large print PDF
Leaving a child with someone else while they are on medicine means two questions have to travel with them: what is due, and when was the last dose given. A one-page log answers both.
What to leave with the sitter
- The medicine, as it is on the label: name, strength, and the instructions as printed. Leave the bottle or box with them so the label is there to read.
- When the last dose was given, with the time, before you leave.
- When the next one is due, as the label or the prescriber says.
- Who to call if something comes up: you, and the number on the label.
What to ask them to write down
- The time of every dose given, and the amount, as on the label.
- Anything skipped, and why.
- Anything unusual they noticed, in a few words.
Keep the log with the medicine, not in a drawer. The next person to give a dose should see the last one first.
How Willowbell does it
A parent adds the child as a person (a first name and an optional age band, never a birthdate), adds the medicine as it is on the label, and taps Take for each dose. As-needed medicines get a "Log a dose" button, so the last dose and its time are always on screen. Print the list to leave with the sitter, or hand over the phone.
Children are entries a parent makes, never users. Willowbell stores what you type; it never calculates a dose or gives medical advice. Reminders are a help, not a guarantee.
Price: Free for one person. A one-time in-app purchase adds everyone else. No account.
- Willowbell for iPhone is coming soon to the App Store.

Willowbell stores what you type and shows it back as a list and a log. It never calculates a dose, suggests a timing, checks interactions or gives medical advice. Strength is shown exactly as you entered it.
